To request a service for a student, navigate to the Request Form in your web browser.
Complete the form, noting that any field marked with a red asterisk is required. The form can only be submitted once all required fields are completed.
Once all required fields are complete, click Submit Request. If the Submit Request button is gray a required field is missing data.
District staff will receive a notification regarding your submitted request. They will review it and contact you if more information is needed.
